To protect your health and the health of its inhabitants, you must complete the electronic form known as “Pase de Salud” or “Health Pass”. From https://www.ministeriodesalud.go.cr/, you can complete your Health Pass and obtain a QR Code to show to airport authorities upon arrival to Costa Rica.
The form must be filled out for every family member, including children and elders traveling with you. This form must be completed 2 days or less before your scheduled arrival.
Fully vaccinated passengers, including children, are no longer required to purchase travel insurance to visit Costa Rica. Those passengers must show their vaccination card or certificate. Vaccines must be administered at least 14 days before arrival. Approved vaccines are Pfizer, Moderna, AstraZeneca, or Johnson & Johnson.
Covid Testing is no longer required to enter Costa Rica.

Requirement for returning to the United States
All air passengers coming to the United States, including U.S. Citizens and fully vaccinated people, must have a negative COVID-19 test result no more than 3 days before travel or documentation of recovery from COVID-19 in the past 3 months before they board a flight to the United States. (CDC)
